All members must have an updated Enrollment Form - Construction on file with the Family Medical Care Plan.  Additionally, a Spouse Employment Data Form is required annually for married members wishing to cover a spouse under this plan.  Make sure to include a marriage license for spouse coverage and birth certificates for any dependent children being covered.
